Every year, Vitalyst assesses opportunities to invest in the catalytic work of community organizations.
Vitalyst Health Foundation is on a mission to connect, support, and inform efforts to improve the health of individuals and communities in Arizona. We achieve this through partnerships across Arizona, including capacity building resources and grant support to organizations who share our mission. Our streamlined “Discovery” process connects collaboratives, nonprofits, and organizational leaders to the best resources to ensure your success improves health outcomes where needs are highest.
Learn more about ways Vitalyst supports community-based leaders, organizations, and coalitions working to improve community health here. Our grant processes are designed to support collaborative projects that are improving the health of Arizonans by making systems more effective, equitable, and sustainable. Depending on the grant type, they range from $10,000-$175,000 and may extend up to three years.
The Vitalyst Health Foundation Discovery Process opened August 1, 2023. It’s the first step in becoming a Vitalyst partner and helps connect partners with appropriate support. The three-step, online process takes between 15-20 minutes to complete.

- The 2023 – 2024 Systems Change Grants Information Session was held Friday, August 11, 2023. To discuss your Systems Change Grant proposal with a Vitalyst team member, complete this Discovery form and note interest in Systems Change Grants, and we will be in contact to schedule a 30-minute conversation via phone call or Zoom. This step is not required in the application process, only if you want to meet and discuss your proposal.

- Submit your Systems Change Grant Letter Of Intent (LOI) here by 5:00 PM (MST-AZ) on October 6, 2023. Email Grants@VitalystHealth.org with questions.
- Spark Grant Applications can be submitted once invited after a conversation with a Vitalyst team member. Instructions for online submission will be shared along with the invitation. Please complete the Discovery Intake Form to meet with the appropriate Vitalyst team member.
