Vitalyst Health Foundation is seeking proposals for the July 2018-June 2021 Vision Medical Assistance Program (MAP).  The goal of the Medical Assistance Program is to improve access to healthcare through direct service for low income individuals, leveraging existing resources and improving the quality of care to underserved populations in Maricopa County.

Proposals are due by 5:00pm AZ time on Friday, March 2nd, 2018 and must be submitted online. The online submission form is here.
